The permit was filed on Aug 12, 2019 specifying a wood-based building with 43 square feet of front yard and 103 square feet of back yard space.
The Brookdale Hospital Medical C filed the permit through their representative John Woelfling of Michael Deruvo & Associates Arch.
The existing building was built in 1899 with 2 floors and 1,825 square feet of space.
